TERMS OF SERVICE UPDATES AS OF 4/20/21 BELOW. PLEASE READ.

IMPORTANT UPDATE as of 4/20/2021

TEST REQUEST FORMS

In order to receive an estimate, the proper Test Request Form must be completed and returned to Manuel Rosa at m.rosa@propacktestlab.com. Send all Purchase orders to Julia Rosa at j.rosa@propacktestlab.com. Incomplete test request forms will cause a delay in the testing process.

Estimates: Updated 4/20/2021

All estimates are just that. An estimate. It is not a formal quote. Due to the nature of the business, it may be necessary to add fees such as labor or charges to re-test upon a package failure which will not be included in the initial estimate. Any additional necessary fees will be discussed with the customer first before being added to the final invoice.

Turn-Around Time as of 4/20/2021

Due to an excessive number of Express Jobs received, our turnaround time has increased to 4-5 weeks from the time all test samples, completed test request forms and a valid purchase order has been received.

This time frame starts when we have received ALL of the following

1) Completed Test Request Forms

2) Testing samples and all related components

3) A valid Purchase Order- Email to J.rosa@propacktestlab.com

If any of the above three are not received or incomplete in any way, testing will be delayed until any issues are resolved.

EXPRESS SERVICES: As of 4/20/21

Express services have been temporarily suspended to the larger than normal number of projects received. In certain circumstances we still offer an express service but please enquire before submitting your paperwork. Any express jobs that can be done will come at a premium cost. We hope to get back to our regular turnaround time of 5-10 business days soon and apologize for any inconvenience this may cause.

Timing of Test Report: As of 4/20/21

The standard turn-around time is for verbal “pass” or “fail” results only. The certified report will follow 3-6 after that.

Shipping of Test Samples

Pro-Pack does not pay for the shipment of any test samples to or from our facility. All collect shipments will be returned to shipper. No Exceptions.

Post Test Procedures:

Once testing is complete all test samples and related materials will be kept for 14 days after which point they will be disposed of unless prior arrangements have been made. It is very important to indicate if the test samples are to be returned on the submitted Test Request Form. If this is left blank on the test request forms, it will be assumed that disposing of the test samples is acceptable. No emails will be sent to verify what is to be done with the test samples following testing.

Payment Terms Updated 4/20/2021

Payment Terms: 2% Net 10 (2.95% Credit Card Processing Fee for all Credit Card Payments)

Payments Types Accepted: Credit Card, ACH and Checks

Note: Any payment terms beyond Net 30 will be assessed an additional charge as follows

Net 60: Add 2% to invoice total

Net 90: add 4% to invoice total

Any unpaid invoices that exceed the payment terms will be assessed a 2% fee (of the current invoice total) per month until paid.
